ACP Journal Club. Review: corticosteroid plus LABA inhalers, vs LABAs alone, reduce morbidity in COPD.
نویسنده
چکیده
Review scope Included studies compared combined ICS plus LABA (ICS/LABA) inhalers (fluticasone and salmeterol [FPS] or budesonide and formoterol [BDF]) with the same LABA alone in adults > 40 years of age who had stable COPD and had not had an exacerbation in the past month. Studies of patients who had partial reversibility on pulmonary function testing were included. Studies that included patients with asthma, cystic fibrosis, bronchiectasis, thoracic surgery, or other lung or significant diseases were excluded. Outcomes included mortality, exacerbation (as defined by individual studies, but usually ≥ 48 h of increased COPD symptoms requiring a change in treatment = moderate exacerbation) rate ratio, number of patients with ≥ 1 exacerbation, pneumonia, and health-related quality of life (St. George’s Respiratory Questionnaire [SGRQ], minimal clinically important difference 10 units).

Comparison and optimal use of fixed combinations in the management of COPD
Chronic obstructive pulmonary disease (COPD) is a leading cause of morbidity and mortality worldwide. Indications for the use of long-acting beta-agonists (LABAs) and inhaled corticosteroids (ICS) in patients with COPD are described in the various international guidelines, but no special recommendations are made concerning the use of combination inhalers containing a LABA as well as an ICS. To ...
متن کاملThe Role of Bronchodilators in Preventing Exacerbations of Chronic Obstructive Pulmonary Disease
Bronchodilators are the cornerstone of symptomatic chronic obstructive pulmonary disease (COPD) treatment. They are routinely recommended for symptom reduction, with a preference of long-acting over short-acting drugs. Bronchodilators are classified into two classes based on distinct modes of action, i.e., long-acting antimuscarinics (LAMA, once-daily and twice-daily), and long-acting β2-agonis...
متن کاملCombination long-acting β-agonists and inhaled corticosteroids compared with long-acting β-agonists alone in older adults with chronic obstructive pulmonary disease.
IMPORTANCE Chronic obstructive pulmonary disease (COPD), a manageable respiratory condition, is the third leading cause of death worldwide. Knowing which prescription medications are the most effective in improving health outcomes for people with COPD is essential to maximizing health outcomes. OBJECTIVE To estimate the long-term benefits of combination long-acting β-agonists (LABAs) and inha...
متن کاملRestriction of LABA use to combination ICS/LABA inhaler therapy in asthma.
In 2011 the British asthma guidelines recommended for the first time that longacting beta agonists (LABAs) should be prescribed in fixed dose combination inhaled corticosteroid (ICS)/LABA inhalers in the treatment of asthma. This represented a revision to the 2009 BTS guidelines in which LABAs were recommended if used with ICS, either as separate inhalers or as a combination ICS/LABA inhaler. T...
متن کاملLABA monotherapy in asthma: an avoidable problem.
an aVoidable probleM Long-acting beta 2-agonists (LABAs) (or beta-adrenoceptor agonists) improve lung function and quality of life, and are recommended at step 3 of the national guidelines for the management of asthma. 1 However, chronic exposure to LABAs can be associated with tolerance and reduced sensitivity to the bronchodilator effects of salbutamol raising concerns over their use in asthm...
